System integration has the potential to realise significant efficiencies whilst also placing the individual at the centre of all services, bringing them the right care and support at the right time, in the right place.

At ICS Assessment Services, we understand that health and social care integration is not just about co-location of services and staff, or an expanded MDT function for complex social care needs. Our Integrated Services utilises the breadth and depth of expertise across the ICS Group to ensure that we can provide sustainable, cost-effective preventative solutions across the health and social care spectrum, placing the person at the centre.

Our approach

Rapid Response Care & Assessment: whether an urgent community crisis requiring complex health and social care support, or a step-down from hospital requires interim community assistance. Our service wraps community nursing, occupational therapy reablement and social care support in with the Authority’s assessment process, by conducting a statutory Care Act assessment alongside short-term support. We then work with your internal teams to find the best long-term solution in line with your budget and capacity.

Front Door Safeguarding Service: we offer permanent qualified triage functions through to full safeguarding monitoring and investigation functions, we can provide a holistic front door service to suit your needs.

Mental Health Assessment & Therapies Service: our team can conduct Section 117 or Mental Health Act Assessments utilising trained AMHPs, qualified Social Work support and RMNs to support a holistic mental health assessment alongside wider social care needs.  In addition, our Digital Therapies service can be utilised to offer a virtual IAPT service at both Step 2 and Step 3, allowing an immediate response to mental health needs whilst progressing the longer-term options.

Discharge 2 Assess and Trusted Assessor models: we provide a range of hospital, community and residential-based assessments, ensuring that the health and social care assessment functions can be carried out independently. Our service offer focuses on a holistic assessment of the individual helping to progress patients in the next stage of their care journey and creating efficiencies in the step-down assessment function.
